Overview

Population Health Management Director oversees operations and reporting of the Nurse Care Manager team, which includes Maryland’s Health Home and Medicare’s Value in Treatment case management programs at multiple clinics. With the aid of appointed team members, this pivotal member of the company’s leadership team requires a dynamic professional who can provide oversight and direction to ensure the delivery of high-quality integrated healthcare services to our clients. 

 

The Director is responsible for strategic planning, growth, comprehensive administration and oversight of the Nurse Care Management team. This includes supervision of off-site and office-based staff, recruiting and retention, development, and implementation of program policies and procedures. In this leadership role, the Director will play a pivotal role in ensuring the delivery of high-quality integrated healthcare services to our clients while fostering a collaborative and supportive working environment for the program's staff.  The Director will also be in charge of implementing new Health Home programs at the Concerted Care Group’s planned additional treatment sites.

Responsibilities

Strategic Planning and Development:

  • Collaborate with executive leadership to develop and implement strategic initiatives for the Population Health Nurse Care Manager Program
  • Assess the evolving healthcare landscape and recommend adjustments to the program to meet changing needs
  • Monitors Population Health performance at the population level and implements improvement effort
  • Program Management
  • Provide overall leadership and direction to the Nurse Care Manager team
  • Monitor and evaluate program performance, ensuring compliance with regulatory requirements and organizational standards, and effectiveness of client care
  • Implement quality improvement endeavors to enhance program effectiveness
  • Spearhead and implement program initiatives, such as health fairs, targeted client education, and other periodic events to enhance client care and promote program growth and awareness
  • Team Leadership and Development
  • Recruit, train, and supervise Nurse Care Manager staff
  • Foster a positive and inclusive work environment, promoting teamwork, communication, and professional growth
  • Develop, review, and implement program policies and procedures
  • Stay abreast of changes in regulations and adjust policies accordingly
  • Collaboration and Networking
  • Build and maintain effective partnerships with external stakeholders, including healthcare providers, community organizations, and governmental agencies
  • Develop and implement strategies to enhance the program's reach and impact in the community
  • Financial Oversight
  • Develop and manage the program budget, ensuring fiscal responsibility and efficiency
  • Data Analysis and Reporting
  • Oversee the collection and analysis of program data, utilizing insights to make informed decisions and drive continuous improvement
  • Prepare regular reports for internal and external stakeholders, highlighting program outcomes and achievements
  • Direct Patient Care
  • Will provide direct patient care as needed in the same capacity as Nurse Care Manager staff

Qualifications

Must Haves:

  • Master's degree in a relevant field (e.g., healthcare administration, public health, social work).
  • Current Maryland State Registered Nurse License in good standing or valid Compact State License.
  • Proven experience in healthcare program management, with a focus on integrated care models.
  • Strong leadership and team management skills.
  • Knowledge of relevant regulatory requirements and accreditation standards.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Demonstrated success in driving program growth and innovation.
  • Ability to analyze and conceptualize data to ensure achievement of desired outcomes.
  • Technically proficient in Microsoft Office programs and database use.
  • Experience working in Behavioral Health and Substance Abuse treatment preferred.
  • MUST be willing to travel up to 60% to our other sites (Hagerstown, Frederick, Cumberland, Baltimore, Fredericksburg, VA)
