Overview

The Fast Track Clinician is a vital member of Kolmac Integrated Behavioral Health, dedicated to providing immediate support and care for individuals seeking help for substance use disorders (SUD). As the first point of clinical contact, this role ensures a seamless intake process, guiding patients through treatment options and motivating them to engage in their recovery journey. This position requires strong interpersonal, organizational, and clinical skills to assess, evaluate, and coordinate care while collaborating with internal teams and external stakeholders.

 

This position is fully remote and will service patients in both Maryland and Pennsylvania. 

Responsibilities

Clinical Evaluations and Patient Assessment

  • Conduct telephonic or online clinical evaluations for patients seeking withdrawal management and outpatient addiction treatment.
  • Assess patients using ASAM (American Society of Addiction Medicine) level of care criteria to ensure accurate placement and treatment recommendations.
  • Identify individual patient needs and establish appropriate treatment plans in collaboration with the clinical team.
  • Utilize evidence-based approaches such as Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), and trauma-informed care to support patients during evaluations.

Documentation and Record Keeping

  • Accurately document clinical evaluations, patient interactions, and treatment recommendations in the electronic health record (EHR) system.
  • Maintain thorough records of communications with patients, referral sources, and internal teams.
  • Complete all required documentation promptly, including clinical notes, treatment summaries, and referral correspondence.

Patient Engagement and Motivation

  • Serve as the first clinical contact for prospective patients, clearly explaining the Kolmac program, treatment options, and what to expect during care.
  • Motivate individuals to commit to treatment and provide reassurance and encouragement to overcome barriers to seeking care.
  • Proactively engage patients to foster trust and promote active participation in their recovery journey.

Collaboration and Coordination

  • Work closely with intake, clinical, and withdrawal management teams to ensure a seamless and efficient admission process.
  • Collaborate with referral sources, family members, and other support providers to ensure continuity of care and comprehensive patient support.
  • Act as a liaison between patients and internal teams, facilitating effective communication to meet individual patient needs.

Outreach and Patient Support

  • Provide follow-up and ongoing engagement to ensure patients remain connected to care throughout the intake and transition process.
  • Coordinate referrals and recommendations for additional support services, such as counseling, medical care, or community resources.
  • Build relationships with referral sources to maintain a steady pipeline of prospective patients.

Special Projects and Additional Responsibilities

  • Participate in organizational initiatives, special studies, and projects to improve service delivery and patient outcomes.
  • Stay up-to-date on clinical best practices, treatment methodologies, and industry standards.
  • Perform other duties as assigned by leadership to support Kolmac’s mission and goals.

 

Qualifications

Education and Licensure

  • Bachelor's degree in a related field.
  • CAC-AD (MD) and CADC (PA).
  • Experience in substance use treatment, behavioral health, or a related field.
  • Proficiency in Motivational Interviewing, Cognitive Behavioral Therapy, and trauma-informed care approaches.
  • Experience working with ASAM level of care criteria preferred.
  • Strong clinical assessment and evaluation skills.
  • Excellent verbal and written communication abilities.
  • Proficiency in using EHR systems and maintaining accurate records.
  • Ability to build trust and rapport with patients from diverse backgrounds.
  • Effective collaboration and coordination skills with internal teams and external stakeholders.
  • Strong organizational and time management skills to handle multiple tasks simultaneously.

The base pay for this position generally ranges between $28 a hour to $31.25 a hour. Factors, such as but not limited to, geographical location, relevant experience, licensure, education, and skill level may impact the pay for this position.
